Preparing for Visit
What to Expect
- New patient visits typically last 45-60 minutes, but can be longer.
- Please arrive 30 minutes in advance of the appointment time if you have not filled out the new patient forms and paperwork online.
Cancelations: Cancelations and changes must be addressed 48 hours before the visit date.
Missed Visits: If you miss 2 or more visits in a row without reasonable explanation, we reserve the right to not schedule a third.
Late Arrivals: Patients who arrive 30 minutes late for a new patient visit or 15 minutes for a follow-up visit may need to wait to be seen. This is to allow patients who arrive on time to be seen without delay.
Medicaid transport will be made available to patients with Medicaid who need transport. Please call 1-866-386-8331 to schedule non-emergency transportation to our office.
Scheduled for a Televisit Appointment? There are two ways to join your scheduled telehealth call:
- Approximately 30 minutes prior to your scheduled telehealth appointment, you will receive an email containing a link to join the video call with your provider. Simply click the link you received from Healow and you will automatically be redirected to the telehealth window.
Visit the Healow Page (https://healow.com/apps/tv/code) and enter the healowTV code you received in your email or over the phone from your provider.
Please remember to bring:
- Any information unable to be completed online such as medical history or family history.
- List of current medications (both prescription and/or over the counter).
- Insurance card
- Identification Card (Driver’s License)
- Co-payment (due at time of service)
Tips for Your Visit:
- Keeping a journal or writing down symptoms and changes as they occur.
- Writing down any questions or concerns and letting our receptionist know ahead of time so that we can address them during the appointment.
If Bringing a Baby:
- Please bring a blanket to keep your child warm during the exam.
For Siblings:
- Please bring homework, books, coloring books, or other activities to keep your child occupied.
